Regular visits from one of our care managers provides peace of mind for the client and their loved ones. Frequency of visits can be tailored to suit the client's needs.  As a third party, working for our clients and their loved ones, we will provide an objective review of the overall circumstances, detect changes that may be occurring or on the horizon, and intervene as needed.

One of our professional care managers visits clients in their home and completes a comprehensive assessment of their living environment, cognitive and physical well-being, and most importantly their wishes and goals for the future.  A detailed  written report,  including a list of recommendations and information about local resources, is provided and discussed with the client, their caregivers, and loved ones.
